Helping you and your family reach and maintain good health is very important to GSI. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
GSI remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.

ELIGIBILITY
All full-time employees working at least 30 hours per week are eligible to participate in the benefit plans offered by GSI. Newly hired employees will be eligible the 1st of the month following 30 days of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
- Medical: Until the last day of the year in which the child turns 26
- Dental and Vision: Until the date the child turns 26
- Dependent Child Life Insurance & AD&D: Until the date the child turns 19, or the date the child turns 26 if a full-time student
Should your employment with GSI end, your coverage will end as of the date of termination.
ENROLLMENT
NEW HIRES! Action Required: You must log into Ease and elect or waive benefits within 30 days of your employment.
ENROLLMENT INSTRUCTIONS:
- Go to gsibenefits.ease.com
- Log in to Ease per the instructions you have received from your HR administrator.
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The only opportunity to modify your plan elections for 2023 is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in Ease within 30 days of the event (60 days if due to adding a new child from birth or adoption). You will need to provide supporting documentation to support the changes based on the Qualifying Life Event. Exception: HSA contributions can be changed at any time for any reason.

Blue Cross Blue Shield of MI (BCBSM) and Blue Care Network (BCN) will continue to be the administrator for you medical and prescription coverage. You have three options to choose from:
- PPO HSA 3000
- In and Out of Network Coverage available
- Member is not required to have a designated PCP to receive in-network benefits.
- No referral is required to see a specialist.
- POS 3000
- In and Out of Network Coverage available
- Michigan Member is required to have a designated PCP to receive in-network benefits.
- No referral is required to see a specialist.
- POS HSA 2000
- In and Out of Network Coverage available
- Michigan Member is required to have a designated PCP to receive in-network benefits.
- No referral is required to see a specialist.

HEALTH SAVINGS ACCOUNT (HSA)

The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- You must be covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a health care Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You must have not received VA benefits in the past 90 days
HSA IRS Maximums for 2023 are as follows:
- $3,850 for a single contract
- $7,750 for a two person or family contract
- $1,000 "Catch Up" contribution for those age 55+
Reminder: You can make changes to your HSA elections at any time for any reason. Please contact Human Resources to request a change to your HSA.
VIRTUAL VISITS

GSI provides those employees who are covered by one of the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provides members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

- Cold/Flu Symptoms
- Sore Throat
- Pink Eye Rashes
- Sinus Infections
- Ear Infections
- Stomach Issues
How Much Does it Cost?
- PPO HSA 3000: 0% After Deductible (Prior to deductible, member will be responsible for a $59 charge that will apply towards deductible)
- POS 3000: $30 copay
- POS HSA 2000: 0% After Deductible (Prior to deductible, member will be responsible for a $59 charge that will apply towards deductible)
- Prescriptions: If you are prescribed a medication during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in and whether your deductible has been met.

GSI's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Be aware if you go out-of-network, your costs will be higher.
Carryover:
If an Insured submits qualifying claims for covered expenses during a benefit year and, in that benefit year, receives benefits that are less than their group’s threshold limit, the insured will be credited a carryover benefit. If an insured reaches his or her certificate year maximum benefit, Unum will pay a benefit from the insured’s carryover account up to the amount stored in the insured’s carryover account. The accrued carryover benefits stored in the carryover account may not be greater than the carryover account limit.
Carryover benefit $250, threshold limit $500, carryover account Limit $1,000.
VISION

GSI offers Vision coverage through Unum to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

GSI provides you with financial protection benefits with Basic Life insurance, Accidental Death & Dismemberment (AD&D) insurance, Voluntary Short Term Disability and Employer Paid Long Term Disability benefits with Unum. You also have the opportunity to purchase Supplemental Life insurance.
Basic Life/AD&D- Employer Paid
You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life insurance benefit is $50,000 (reduced if you are age 65 or older), and the amount of your AD&D benefit is $50,000. You do not need to enroll in these benefits as they are provided at no cost to you.
Supplemental Life- Employee Paid
You have the opportunity to enroll in Supplemental Life insurance for yourself, your spouse or your dependent children, which will supplement lost income in the event of death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. You must elect Supplemental Life insurance coverage in order to elect coverage for your spouse and/or children.
- Employee: Choose in $10,000 increments up to the lesser of 5x your annual salary or $500,000. Guarantee Issue: $100,000
- Spouse: Choose in $5,000 increments up to 100% of the amount you elect for yourself or $250,000 Guarantee Issue: $25,000
- Child: Choose in $2,000 increments up to $10,000 *Live birth to 6 months, maximum benefit is $1,000. Guarantee Issue: $10,000
Evidence of Insurability (EOI): When you are first eligible (at hire) for Voluntary Life, you may purchase up to the Guaranteed Issue without Evidence of Insurability (EOI). Any amount elected over the GI will require EOI.
Future Increase Option: During the annual open enrollment you may be eligible fo increase your supplemental life insurance with no medical questions! If already enrolled, employees may increase up to the Guarentee Issue without EOI. Any amount elected over the GI will require EOI..
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short Term Disability- Employee Paid
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the ability to purchase Short Term Disability coverage that will provide you 60% of your weekly income (up to $1,500 per week) for up to 25 weeks.
Typical claims would include: pregnancy, injuries, joint, back and digestive disorders.
Long Term Disability-Employer Paid
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. GSI provides you with a Long Term Disability benefit, that covers 60% of your monthly income up to a maximum benefit of $5,000 per month. Payments will last for as long as you are disabled, or until you reach Retirement Age (age 65), whichever is sooner.
Typical claims would include: cancer, back disorders, injuries and poison, cardiovascular, joint disorders.
